Please print and complete this form to order your Creative Tables, Inc. Swatch Box. The box, available to the trade only, contains over 100 6”x6” fabric swatches, applicable pricing and order information. You may either return the form to the address below along with your check for $60.00 payable to Creative Tables, Inc. or fax the completed form with your credit card information and authorization to our office. Your Swatch Box will be shipped within five days of receipt of payment via Fed Ex ground. An additional charge will apply for air shipments. Updates to the swatch box will be sent as additional linens are added to our inventory.
